It seems that HealthForce is continuing to present forms that request you to release all your medical history to be shared with the company. By law, the company is required to maintain a copy of your current DOT card. They can request your medical long form, however no law requires this, nor does it require you to allow it. When you acquire your new card, be sure to ask what the check boxes mean. Some of the HealthForce staff are under the impression that Gray Line will not pay for the physical if you do not authorize the release. Should they insist you mark such a box, you have three options.
1. Ask them to call Jay Rogers to authorize payment. Jay will remind you that you are required to provide a photocopy upon receipt of your new card.
2. Schedule another visit, and coordinate that visit so Jay can clear your way.
3. Pay for the physical yourself and submit an expense statement for reimbursement.
Jim Colwell has held fast regarding the long forms to be presented to the Company. The liability of holding such information does not seem to deter his opinion. Jeremy Butzlaff and Jay Rogers understand the liability.
