When you visit HealthForce for your bi-annual DOT Physical, the release form will look a little different than in past visits. In addition to the check box for all medical records related to the physical, there are several "Optional" check boxes for information related to specific medical events.

Chairman Schneider received confirmation that we no longer have to allow the physical long form to be transmitted to Gray Line, nor do we have to pay up front to avoid the long form being sent to Gray Line. Personally, I find the release form to be little confusing, as it appears that you must check the first box and all subsequent boxes are "optional". It is my understanding that we need not check any boxes, therefore the long forms will not be forwarded to Gray Line.

Gray Line is entitled to, in fact required by law, to maintain a copy of your DOT Certification card. This card indicates that all medical records accumulated for the purposes of completing said certification are on file at the Doctor's Office, where they should be!

If you have any questions or difficulties in obtaining your certification card without allowing long forms to be forwarded to Gray Line, please bring this to the attention of one of your Union Representatives as quickly as possible.
